A $100.00 deposit is due at the signing of this contract, payable to TJ MARTINEZ.

Terms of Agreement

  1. Minimum deposit of $100.00 required to bind terms.  Check should be payable to TJ MARTINEZ.
  2. Any deposit received in non-refundable.
  3. Parties entering into this contract are permanently bound to fulfill the obligations by this agreement unless one or more of the following conditions apply:
    1. Both parties mutually agree to leave the agreement and its responsibilities.
    2. The event described herein is completely cancelled, and a minimum of 60 days notice is given.
  4. In the event that Tj Martinez becomes unable to provide his services due to complications beyond his control, a reputable replacement shall be provided at no more than the original price agreed on.
  5. Any and all information or details must be communicated before the 5 days preceding the performance.
  6. Tj Martinez shall not be held liable for any action arising from or in connection with any individuals not directly affiliated with Tj Martinez.  Furthermore, the party(s) contacting Tj Martinez agrees to assume full responsibility for any and all damages caused by themselves or their guests involving any of the Tj Martinez property.
  7. The parties purchasing the services of Life of the Party/LOTP agree to allow Life of the Party/LOTP to use pictures and video images taken at this event for promotional purposes
